• Place of Legal Residence:

• Survivors Planner: Planning For Your Survivors
• Marital Status with Spouse’s Full Legal Name Including Maiden Name (If Applicable)
• Race
• Education
• Fathers Name
• Mothers Name with Maiden
• Armed Forces Information, Including DD-214 (If Applicable)
• Occupation Title, Kind of Business/Industry
• Informant’s (Legal Next of Kin) Name and Address
• Photo(s) of your loved one
